The contact form collects only your name, email address, and message, which are delivered by email to Bright Plate Nutrition so we can reply. Please do not include personal health details in the form; those belong in your secure intake form and appointments, which are handled through a separate, privacy-compliant practice platform.
Downloadable handouts require no sign-up and no personal information. This site does not run advertising.
